The Care Planning module allows for easily created Care Plans that are fully integrated with MDS (RAPs) and Progress Notes. Momentum’s Care Plan Library has been developed based on documentation in the RAP guideline section of the RAI Version 2.0 manual, as well numerous other research-based resources.
