MDS for Home Care is designed to automate the collection, calculation and electronic transfer of assessment data and CAPs (Client Assessment Protocols). Case management capabilities allow for client allocation to a case coordinator. The automated assessment appears like the paper-based tool and is an easy-to-use and affordable Windows-based product.
Our check-out and check-in feature allows for the completion of an assessment in a client’s home. Client information is transferred from a central server to a local system, such as a notebook or handheld device. Then, following an off-site or bedside assessment, the client data is electronically updated and archived at the central server.

The MDS management solution is an electronic Minimum Data Set tool used for collection, analysis and submission of client assessments resulting in:
- Decreased staff time spent collecting and entering data
- Optimized RUG III scores
- Easy inter-organization and intra-organization analysis of Quality Indicators, CAPs, and Outcome Scales
- Alerts warn of incomplete sections
- Notes can be entered and viewed or printed out as a care plan
Security is strong and flexible and can be configured so that designated users are able to modify, complete, view or lock assessments. Sections are digitally signed by the users who complete them, resulting in full audit logging.
CAPs are automatically calculated as the assessment is completed. Visual cues in the assessment alert the user to the CAP that has been triggered. All CAPs can be reviewed with drill down capability from one single summary screen.
Client Allocation and Checkouts – Clients can be allocated to a case coordinator and case loads can be transferred. Checking out allows for bedside or off-site assessment.
Electronic Data Submission is available by disk, modem or Internet.

- Client Demographics and Contacts
Certain reports are client specific, providing detailed information for individual clients. Other reports summarize information for all clients on RUG and Outcome scores, last assessment dates and when assessments are due to assist with workload management and scheduling.
Lookup Tables for ICD9/10 diagnostic codes and Drug Identifier Numbers (DIN) are incorporated into the assessment sections to speed data entry.
