Care Planning
Effective case management is the cornerstone of client care. Through the use of the Care Planning module, care providers can design a consistent and purposeful strategy which acts as a case framework. This framework consolidates and coordinates care providers’ efforts to afford the best care for a client.

Care Plans are used to track a client’s progress throughout the history of a case. Each registration can have several Care Plan records attached, tracking the status of a client at different points in time. Care Plans can also be linked to Request and Diagnosis records; detailed information is only a click away.

The Care Plan can has a defined purpose and goal. The clinician responsible for the plan is documented and a team of care providers can be assigned to carry out the plan. Tests can be chosen from a facility-defined list and client scores recorded. Subtests and scores can also be recorded. Free-form fields, like Need/Problem and Note, allow clinicians to record additional information which may be pertinent to a Care Plan. At the completion of the plan, a final outcome can be recorded, as well as a date for a follow-up measurement.

Teams
Teams are combinations of care providers as defined by the facility. Teams are linked to a service type and can be assigned a default care provider and a manager. One or more teams can be specified for each registration, if appropriate. Review a client’s list of teams and sort by service type, team member, status or manager.
