Special Open Door Forum: MDS 3.0

The Update
CMS held a Special Open Door Forum on the MDS 3.0 January 24, 2008 and Momentum Healthware participated in the teleconference, where attendance over the phone alone was over 2,000 people.

A presentation of the relevant research and rationale behind the new assessment design was provided by RAND Health lead researcher Dr. Debra Saliba. The 5 main areas of format change within the assessment were outlined, which included: Cognitive Assessment, Mood Assessment, Behaviour items, Customary Routine and Activities, and Pain. The inclusion of tested gold standard assessment instruments in the development of these redesigned assessment sections was discussed and includes contributions from the Brief Interview for Mental Status (BIMS), the Confusion Assessment Method (CAM) and the PHQ-9 (Mood assessment).

Emphasis was placed on improvements the MDS 3.0 will bring to resident care over the current 2.0. The new interview format will bring increased clinical relevance to the assessment, as well as clarity and efficiency. There is also increased reliability and validity of the new MDS 3.0 as well as the decreased in average completion time for the full assessment by 45% (62 minutes for the MDS 3.0 compared to 112 minutes for the same assessment in the MDS 2.0). This did not include care planning or working the RAPs.

The proposed timeline for the implementation of the new MDS 3.0 was also reviewed. A MDS 2.0/MDS 3.0 crosswalk to states, providers and vendors is to be posted in the spring of 2008. Once the STRIVE (Staff Time Resource Intensity Verification) study has been completed, further research will be needed to recalculate RUG values and quality indicators. This information will be included in final specifications to providers and vendors in February 2009. Final implementation of the MDS 3.0 will occur on October 1, 2009 and it will be utilized in the survey process also in October 2009.
