Care Planning
The Care Planning module allows for easily created Care Plans that are fully integrated with MDS (RAPs) and Progress Notes. It includes a library of standards-based Care Plans which can be quickly individualized or you can add custom Care Plans. Momentum’s’ Care Plan Library has been developed based on the standards of the Joint Commission of Accreditation Healthcare Organizations, as well other research-based resources.
